Matching Gift data provided exclusively by
Obtain your company's matching gift application form from your company's human resources office. Complete the first part of the application form (donor and gift information) and send it to the Foundation for Student Achievement with your gift. Please note: You can complete a matching gifts application form for gifts made by credit card or over the Internet, or for gifts you have made earlier in the year as well. The foundation will complete the gift certification on your application and send it to your company to receive your matching gift.
